As an Otolaryngologist, now part-time and moving away from active practice, I am CEO of www.nomoreclipboard.com, a personal health record company.  NoMoreClipboard is a spin-off of Medical Informatics Engineering, a Fort Wayne, Indiana company of about 14 years experience in hosting the MedWeb, a Northeastern Indiana regional health network.  MIE created a natively web-based platform that supports electronic health records and, using the same broadly enabled platform, a PHR: NoMoreClipboard, (NMC.)
 

We use NMC both as a consumer product (available on line at our website, through Google Health, and through HealthVault) and as an informatics tool providing PHR services through University Student Websites, Hospital Portals, and Physicians' branded websites.
  

We have pilot programs in progress with CMS, for Medicare patients (Arizona & Utah) and for safety-net programs in which our PHR is used for indigent and homeless patients (Howard University & Fort Wayne HAP program.)
